Job Description:

One Wight Health is our Islands GP Federation. Working closely with our 12 General Practitioner (GP) practices on the Island we ensure we meet the changing health care needs of our local population., A key part of these roles will be to support individuals to get to the right services and to feed back to the wider project team with information which will in turn support their health care plans to improve overall health and wellbeing.

Job Responsibilities:

  • Keep accurate records/data collection as required by project leaders
  • Provide direction and support to enable patients to access services if required
  • Be pro-active in building good relationships with practice colleagues, other service providers including community mental health team and the voluntary sector
  • Participate in regular team meetings to ensure information about all services is documented and shared
  • Identify Patient support needs and sign post to relevant services
  • Follow up calls/ provide updates to services and sign posting as needed
  • Provide specific support to the Social Prescribing Team and Mental Health Practitioners on designated casework as assigned
  • Build working relationships quickly and effectively within our Member Practices and external stakeholders instilling confidence and maintain existing relationships
  • Maintain strict confidentiality at all times and awareness of GDPR/ data sharing throughout working practices

Job Requirements:

  • Managing clear and consistent boundaries
  • Excellent time management skills – ability to prioritize, plan and deliver work items in line with set expectations
  • Confidence to escalate or refer work flow where needed / identify risk
  • Ability to remain calm and professional at all times
  • Ability to work cohesively as part of a wider supportive team whilst maintaining the confidence to work independently on agreed workflow items within remit
  • Confidently communicate at all levels to include Medical Professionals, Commissioners and Governing bodies (where appropriate)
  • Strong communication skills both via telephone, face to face and electronically (as well as the ability to adapt communication style to meet the needs of an individual as identified)
